Late of Gayndah.  Passed away peacefully on Thursday, 23 June 2016, aged 91 years.

Much loved Husband of Bethel (dec’d).  Adored Father of Chris.

Relatives and friends are respectfully invited to attend a celebration of Jack’s life to be held at St Josephs Catholic Church, Gayndah on Friday, 1 July 2016, commencing at 10.30 am, followed by interment at the Gayndah Lawn Cemetery.
